How does the Plan use the power of attorney?


When we receive the copy of the Power of Attorney for Property, along with the completed and signed Confirmation of Annuity Payment from the agent, we will add them to your file. We require that the Confirmation of Annuity Payment be completed and returned to us annually. We do not require another copy of your Power of Attorney for Property as long as it remains in effect. The holder of your current and enforceable Power of Attorney for Property simply checks the box on the form, indicating that they have already sent it and that it remains in effect.
